Does your husband has a medical insurance? If he has, it is no questions -- do it in the US.

If he does not, a number of states offer free care programs. In those states it is still much better in the US.

Benefits.

1. The child is a US citizen.
2. Medical conditions are much better.

Disagvantages.

If there is no medical insurance, you will have a large debt and a very bad credit history.

About the free care programs. I am talking about free care program in Massachusetts. It is neither federal nor state assistance. The program encourages hospitals (private and state ones) to spend some money for treatment of people who cannot pay. This program is available to everybody, including illegal aliens, who does not have alien number or social security number. Almost all hospitals in Massachusetts participate in the program, including 2 best ones, which are ranked among first 10 in the US, where my son was born (I had an insurance, though).

When my cousen was immigrating from Israel to the US on a DV green card, I specifically asked in both of them about delivery (they were parents-to-be) in terms of free care. They told me they would definitely allow my cousen's wife to have a delivery there (Brigham & Women's Hospital, rank #4 in the US in the area of obstetry/gynecology) under free care program.

She just needed to come to the hospital emergency room for delivery.

Actually, they were afraid to fly 4 weeks before the delivery (it is about 6 hours of flying over the Ocean with no possibility to have an obstetrician on board), so they had a baby in Israel and came here later.

But I worked out the question very well.

There is absolutely no risk using it either before applying for green card, or after that.
